Education, leaving care, falls and later life

These pages and sections appear only when your organisation has switched on the matching Care feature: Education, Child care or Elderly care. Normal visibility still applies: you only see people in homes you have access to.

Education

Where: profile → Education. Who: view care.education.view; change care.education.manage. Needs the Education feature. Education is restricted information, and every view is logged.

A person has at most one current education enrolment: the school, college or course they attend now. Attendance and results are recorded against it, per period such as a term.

Only one current enrolmentThis beneficiary already has a current education enrolment; end it first

Changing school: select End on the current enrolment and answer "Why is this enrolment ending? (it ends today)", for example "Moved to Oak Academy". Then start the new one. Correct fixes a mistake on the current enrolment without ending it.

Attendance and results

Select Add record on the enrolment:

Select Save record. Use Edit on a row to correct it. The table shows Period, Attendance, Results, Exclusions and Notes. A period can only be recorded once per enrolment.

There are no timetables, report-card files or links to school systems.

Leaving care

Where: Care → Leaving care. Who: beneficiary view. Needs the Child care feature.

Young people leave children's care at an age limit. The page lists who reaches it soon, so staff can plan their move to independence in good time.

How the leaving date is worked out

Example: Daniel was born on 15 March 2009, in Children residential care (maximum 18). He leaves care on 15 March 2027.

Use the worklist

  1. Choose a Facility (or All facilities I can see) and a window: Within 6, 12 or 24 months.
  2. The list is ordered soonest first, and includes anyone already overdue. Columns: Beneficiary, Leaves care on, Age limit, Months left, Pathway.
  3. Pathway shows Started when the person's care plan has an open goal in the Independence area. Otherwise it shows No independence goal.
  4. Open the person and add an Independence goal to their care plan, such as "Learn to budget and cook", with actions.

Alerts on the profile

WhenAlert
12 months or less to goA warning: leaving care due
6 months or less to goA stronger warning

Nothing changes automatically: no goals are added and nobody is notified. Staff plan the move.

People with no date of birth, or no age limit, can't be listed. Check the Party's date of birth and the programme's maximum age.

Life-story items

Photos and letters that belong in a young person's life story or memory book are document records with Life-story or memory-book item ticked. Only Photo, Correspondence and Other documents can be life-story items.

Later life

Where: profile → Later life. Needs the Elderly care feature. The section brings together what matters when supporting an older person.

Functional summary

The latest completed assessment for each area, from templates that have a Later-life summary area set (see templates). Needs care.assessment.view.

AreaAssessmentDateResultNext due
Falls riskMoving and handling risk v22026-01-10High risk: refer to physio2026-04-10
NutritionEating and drinking check v12026-02-01Low risk2026-05-02

If an area is missing, no completed assessment exists for it. Complete one rather than writing a summary by hand.

Cognition and mental health

Active mental-health conditions from the health record, such as "Alzheimer's disease · Active · since 2024-05-01". Needs care.health.view. They are recorded in the health record.

Falls

"3 fall(s) in the last 90 days", with a link to the Falls register. Two or more falls in 90 days also raises a repeated falls alert on the profile.

End-of-life wishes and advance directives are recorded under Health; attorneys and next of kin under Legal authority and consent.

What matters to me

The person's own preferences, in their own words.

FieldWhat to enterExample (Ruth)
CategoryWhat matters to me, Daily routine, Food and drink, Faith and culture, Communication, Activities and interests or Personal careDaily routine
Who said itSelf, Family, Advocate or Staff observationSelf
Review onOptional review date01/07/2026
In their own wordsQuote the person"I like a cup of tea in bed at 7, and I don't want to be got up before 8."

Select Save preference.

Changing a preference: select Replace on the current one, or record a new one in the same category. The form shows 'This replaces: "…"'. The old one moves to the history, marked "replaced". Preferences are never edited in place, so you can always see how they changed.

Faith and culture is the person's own practice, for example "Attends church on Sundays; likes to listen to hymns". It is not the special-category religion field in Origin and background.

Alert: a preference with a Review on date within the next 30 days raises a review-due alert.

Falls register

Where: Care → Falls register. Who: care.incident.view. Needs the Elderly care feature.

  1. Choose a Facility (or All facilities I can see) and a Period: last 30, 90 or 365 days.
  2. The list is ordered by most falls first, with columns Resident, Falls, Last fall (a link to the incident) and Falls risk (the latest Falls risk assessment, or "None recorded").

Falls come from Fall incidents. The register only counts incidents you are allowed to open, so two colleagues may see different numbers.

Example: Elm Lodge, last 90 days: Ruth Evans has 3 falls, the last on INC-000027, and Falls risk "High". Tom reviews her moving and handling plan.
